If you have already reported the loss or theft of your mobile phone to the local authorities, go back and ask them to withdraw the report. If you have already written out a self-declaration, compile a recovery self-declaration form. Fax a copy to the 800.034651 toll free number. Make sure that the block reference number given to you by IMEI is indicated on both the Police report and the self-declaration.
